What data do we collect and why?
We collect your email address and contact number only for the purpose of your appointment. This may be to:

To confirm your booking.

To remind you of your appointment.

To arrange a remote or telephone review to discuss the results of your sleep test.

Sleep Medical will ask you to complete a series of questionnaires which will contain information such as your name, date of birth, estimated height, weight, collar size, occupation, driving status, medication, caffeine intake, your level of daytime sleepiness. You may be asked to complete another questionnaire which will provide a measure of your quality of life. This information will only be used for the purpose of the clinical report.

After we download your sleep device, we will obtain important parameters which are required for the purpose of the clinical report such as your oxygen desaturation index, pulse events/rises and the mean nocturnal oxygen level.

We use your information in accordance with the data protection principles and comply with data protection law. The personal information we hold about you must be:

Collected only for valid purposes that we have clearly explained to you and not used in any way that is incompatible with those purposes

Use of Cookies
We may use a “cookie” in order to track website usage. These cookies do not collect personal information, but records the time of visit, date, referring page, and the IP address. This information is used by us to create a better user experience.

How long do we retain your information?
We will only retain your personal information for as long as necessary to fulfil the purposes we collected it for or until you withdraw consent.

You can withdraw consent at any time by emailing sleepmedicalni@gmail.com. Once we receive notification we will withdraw we will no longer process your data for the purpose or purposes you had originally agreed to.

How is my information deleted?
Once your information is held for as long as necessary, it is completely deleted from the computer and all back-up files from the recycle bin. All questionnaires are immediately shredded on site.

Sleep Medical have implemented a number of procedures to deal with any suspected data security breach. We will notify you and any applicable regulator of a suspected breach where we are legally required to do so.
